Two things get conflated on almost every page that answers this question. Clearing alcohol from the body takes hours. Repairing the liver takes weeks to months. At some stages it does not happen at all.
The intervals below apply to a liver at the fatty stage, hepatic steatosis. That is the earliest form of alcohol-related liver disease (ARLD), and NHS inform describes it as broadly reversible once drinking stops. Once fibrosis is established, the timeline stretches out and partly stops applying. Once cirrhosis is established, the scarring is permanent.
Every figure here is a population average from published research. None of it is a forecast for one person. Age, sex, years of drinking, BMI, hepatitis B or C co-infection and genetics all move the number.
The safety point belongs at the top, not buried in an FAQ. In a physically dependent drinker, stopping abruptly can trigger alcohol withdrawal syndrome: tremor, agitation, seizures, and in the worst cases delirium tremens. NICE clinical guideline CG100 and NHS guidance both send heavy daily drinkers toward medically assisted withdrawal rather than a cold stop. NICE also advises thiamine for people at risk of Wernicke’s encephalopathy, a nerve and brain injury driven by vitamin B1 depletion. If you drink daily and have ever shaken in the morning, the first step is a doctor, not a calendar.
What alcohol does to the liver first
Ethanol is broken down inside the hepatocyte in two steps. Alcohol dehydrogenase (ADH) turns ethanol into acetaldehyde. Aldehyde dehydrogenase, mainly ALDH2, turns acetaldehyde into acetate, which the body handles easily.
Acetaldehyde is the toxic middle step. It binds proteins and DNA. It piles up whenever intake outruns enzyme capacity, which is why the speed of drinking matters and not only the total. The International Agency for Research on Cancer classifies both alcohol and acetaldehyde from alcoholic drinks as Group 1 carcinogens.
Fat accumulates for a separate reason. Both oxidation steps convert NAD+ to NADH. The shift in the NADH/NAD+ ratio suppresses fatty acid oxidation and pushes the liver toward making triglycerides. That is the documented mechanism behind alcoholic steatosis. It is also why fat can appear in the liver of someone whose diet has not changed at all.
The word “detox” is worth retiring here. There is no cleansing process to buy. There is hepatic metabolism, which either keeps up or does not.
Regeneration is real but conditional. Hepatocytes replicate and replace themselves. The collagen scaffold laid down by activated stellate cells does not clear the same way. That single difference is why the answer changes so sharply between the fatty stage and the fibrotic one.
One modifier that no ranking page mentions: the ALDH2*2 variant slows acetaldehyde clearance. It is carried by a large share of East Asian populations and is present in parts of Northeast India, while staying uncommon in most South Asian groups. Carriers accumulate more acetaldehyde per unit of alcohol.

Days 1-3: acetaldehyde clears
Ethanol is gone within hours of the last drink. Most adults clear roughly one UK unit an hour. Acetaldehyde follows quickly once no more is being made.
What resolves over the next day or two is the metabolic disruption. The NADH/NAD+ shift reverses. Gluconeogenesis returns to normal. Fluid and electrolyte handling settles.
Nothing shows on a blood test this fast. Do not book an LFT on day three and read it as a verdict. ALT has a serum half-life of about 47 hours and AST about 17 hours, so even at best the numbers still describe the last few days of injury.
This window is also the dangerous one. Withdrawal symptoms begin 6 to 24 hours after the last drink. Seizures cluster in the first 48 hours. Delirium tremens usually appears at 48 to 72 hours, and NICE CG100 treats it as a medical emergency. Claims about energy, sleep or “clarity” in this window are not liver findings, and they are left out here on purpose.
Weeks 1-2: what blood tests start to show
A standard liver function test (LFT) panel in India reports ALT (SGPT), AST (SGOT), GGT, ALP, total and direct bilirubin, and albumin. ALT and AST leak out of damaged hepatocytes. GGT is the marker most responsive to recent drinking. ALP and bilirubin point toward bile flow, and toward advanced disease when they rise alongside jaundice.
GGT falls first for a plain pharmacokinetic reason. Its biological half-life runs 14 to 26 days. A raised GGT in someone who stops drinking should sit visibly lower on a repeat test at four to six weeks. So test at baseline, test again after a month, and compare the two.
An AST:ALT ratio above 2 is a recognised pattern in alcohol-related injury, described in EASL’s ARLD guidelines. It is a clinician’s signal, weighed against history and imaging. It is not a self-diagnosis tool. Plenty of alcohol-related injury never produces it.

An LFT panel is one of the cheapest tests on any Indian diagnostic chain’s rate card. Prices differ by state, city and provider. Check the current rate card of the lab you plan to use rather than trusting a figure from an article.
One caveat outweighs the rest. Normal enzymes do not mean a normal liver. Fibrosis can sit behind an unremarkable LFT. That gap is exactly why elastography exists.
Weeks 2-4: the fat reduction window
The most-quoted evidence comes from a prospective study by Mehta and colleagues at the Royal Free Hospital and University College London, published in BMJ Open in 2018. Ninety-four moderate-to-heavy drinkers abstained for one month. A control group carried on drinking.
Liver stiffness on transient elastography fell by around 12.5% on average. Insulin resistance fell by roughly a quarter. Blood pressure and circulating cancer-related growth factors dropped too.
Read the population before you quote the number. The participants were largely healthy volunteers. They were not alcohol dependent and had no diagnosed liver disease. The result tells you what one month does to a mildly injured liver. It says nothing about a dependent drinker with alcoholic hepatitis.
Weight loss during an abstinent month is a calorie effect. Ethanol carries about 7 kcal per gram, so a 60 ml bada peg brings roughly 70 kcal before mixers. Losing weight helps the liver. It is a downstream consequence, not the mechanism.
One month is the standard research window for a mundane reason. Dry January, run by Alcohol Change UK since 2013, created large groups abstaining on the same schedule, which made recruitment easy. A study design became a campaign. Is one month enough? At the fatty stage it can produce real improvement, and going back to previous intake gives the gain straight back.
Months 3-12 and beyond
Past three months the question shifts from fat to scar. EASL’s guidelines describe fibrosis regression as possible with sustained abstinence, and less likely as the stage advances. Early fibrosis has the best chance. Bridging fibrosis has less.
Tracking it means repeat elastography or a serial FIB-4 score, usually at 6 to 12 month intervals. FIB-4 is calculated from age, AST, ALT and platelet count. A score under 1.3 makes advanced fibrosis unlikely in most patients; a score above 2.67 raises the suspicion sharply. Expect to be measured over a year, not a month.
For patients who already have cirrhosis, abstinence remains the single intervention with the clearest effect on survival. Cohort data summarised by EASL show better survival in abstinent cirrhotic patients than in those who keep drinking, at both compensated and decompensated stages. Abstinence is also a formal requirement in most transplant pathways, where listing decisions lean on the MELD score.
The trajectory is never uniform. Years of intake, weekly volume, sex, BMI, hepatitis B or C co-infection and genetics all shift it.
What quitting cannot undo
The recognised progression of ARLD has four steps: hepatic steatosis, alcoholic hepatitis, fibrosis, cirrhosis. Descriptions that skip alcoholic hepatitis are incomplete. It sits between fatty liver and scarring, and the severe form can kill.

NHS inform describes steatosis as broadly reversible. Alcoholic hepatitis can resolve in mild cases and carries high mortality in severe ones. Fibrosis may partly regress. Cirrhosis does not reverse. The architectural distortion and regenerative nodules stay for life, and that is the ceiling on any repair timeline.
What abstinence still buys at that stage is not small. Progression slows. Decompensation events such as variceal bleeding from portal hypertension, ascites and hepatic encephalopathy become less likely. Survival improves. Transplant assessment becomes possible.
One risk persists regardless. Cirrhosis from any cause remains a risk factor for hepatocellular carcinoma after drinking stops. EASL and AASLD both recommend six-monthly ultrasound surveillance in cirrhotic patients indefinitely. Stopping lowers the trajectory. It does not return the risk to baseline.
How much drinking gets you here
Indian drinkers count in pegs, not US standard drinks. Most Indian-made spirits are bottled at 42.8% ABV. Imported spirits are commonly 40%. Check the label, because ABV varies by brand and batch. Grams of ethanol are volume × ABV × 0.789 g/ml.

Thresholds vary by body. EASL uses daily intake above 30 g of ethanol for men and 20 g for women as the point where ARLD risk climbs. The UK Chief Medical Officers’ 2016 guidance sets 14 units a week, about 112 g, for both sexes. The US NIAAA counts one standard drink as 14 g and calls it heavy drinking above 14 drinks a week for men and 7 for women. The CDC defines binge drinking as 5 drinks for men or 4 for women within about two hours. The World Health Organization went further in January 2023: no level of alcohol consumption is safe for health. ICMR-NIN’s 2024 Dietary Guidelines for Indians advise avoiding alcohol.
Translate it. Three bada pegs on each of three nights a week is about 183 g of ethanol, roughly 23 UK units. That is well past 14 units, and past the EASL daily threshold on the nights you drink. How to find out where your liver stands
The escalation ladder is short. LFT panel first. Ultrasound if enzymes are raised or symptoms warrant, remembering that it detects steatosis but not early fibrosis. Then FibroScan, meaning transient elastography, or a FIB-4 score from routine bloods. Hepatologist referral for anything abnormal.

Elastography is available at dedicated liver centres, including the Institute of Liver and Biliary Sciences (ILBS) in New Delhi and AIIMS New Delhi, and at large private hospital chains in most metros. Tell the doctor three things: honest weekly volume in pegs, how many years at that level, and any previous abnormal LFT. Under-reporting is the most common reason a raised GGT gets blamed on something else.
Four symptoms mean go now, whatever the timeline says. Yellowing of the eyes or skin. Swelling of the abdomen or legs. Vomiting blood or passing black stools. Confusion or drowsiness. NHS inform lists these as signs of advanced ARLD.
Home test strips and wearable trackers cannot assess fibrosis. They replace neither the blood test nor the imaging.
If you are cutting down rather than quitting
Cumulative weekly volume is the main driver of risk. Pattern is not neutral either. A Danish cohort published in the Journal of Hepatology in 2015 reported higher cirrhosis risk with daily drinking than with the same weekly amount spread over fewer days. That is the evidence base behind alcohol-free days. It supports fewer drinking days at lower total volume. It does not support the same volume squeezed into a weekend.
Substitution is now practical in India. Under FSSAI’s standards for alcoholic beverages, products at or below 0.5% ABV fall outside the alcoholic beverage definition and are sold as non-alcoholic. Availability is uneven. State excise rules, licensing and dry days differ across India, and a dry days by state list is the quickest way to check yours. Worth knowing too: the 2023 renaming of NAFLD to MASLD introduced a MetALD category, covering people with metabolic risk factors plus higher alcohol intake. The two causes of fatty liver overlap constantly.
